Knee Pain
- Mechanical fall down several stairs landing on left knee
- Pain in knee worse with weight bearing
- Pain worsening over time
Compartment Syndrome
- The 6 P's - Pain, Paresthesia, Pallor, Poikilothermia, Paralysis, Pulselessness
- Lower leg has 4 compartments
- Anterior - anterior tibial artery and deep peroneal nerve
- Lateral - superficial peroneal nerve
- Superficial Posterior - sural nerve
- Deep Posterior - posterior tibial artery and nerve
Evaluation
- Normal compartment pressures = 0 - 10 mmHg
- Blood flow becomes compromised at compartment pressures > 20 mmHg
- (Diastolic BP) - (Compartment Pressure) < 30 mmHg = Likely Compartment Syndrome
- High index of suspicion - discuss with surgery / ortho
